Summary
We are looking for an energetic, highly motivated Clinical/Counselling Psychologist or CBT Therapist to join our dynamic Diabetes Psychology Team.
If you are an enthusiastic and motivated clinician with experience in working with people with diabetes or Long-Term Conditions (LTC), and you are passionate about delivering evidence-based psychological interventions and service development, we encourage you to apply.
This is an excellent opportunity to develop advanced clinical skills within a multidisciplinary team and be involved in a key service development project. Regular supervision will be provided.
The post holder will be responsible for delivering a specialist psychology service to patients living with Type 2 diabetes in Well Street Common PCN and Hackney Marshes PCN.
Your work within the Diabetes Psychology team will be central to improving patient engagement, self-management, and overall outcomes, with a specific focus on addressing health inequalities. This includes identifying and working with patients who struggle to attend appointments, as well as providing one-to-one and group-based support for individuals struggling with diabetes related distress. It will also involve wider outreach in the community and skilling up primary care staff to improve effectiveness of patient contact.
You will work alongside a multidisciplinary team with disciplines including Endocrinology, Nursing, Psychology, Dietetics, and Diabetes Lay Educators and the wider primary care team.
• Clinical: Providing specialist psychological interventions to complex patients living with Type 2 diabetes, within HM & WSC PCNs to improve self-management and promote active, healthy lifestyles.
• Service & Community Engagement: Identifying barriers to engagement in diabetes management within the two PCNs through focused research, including questionnaires and focus groups.
• Awareness & Outreach: Engaging with the community to raise awareness of diabetes management resources, with a strong focus on marginalised groups.
• Staff Training & Support: Skilling up existing staff on the knowledge and awareness of psychological barriers to engagement in diabetes care, and utilising resources like MDMs to improve the effectiveness of patient contact and reduce disengagement.
• Peer Support: Setting up peer support initiatives based on identified need, making strategic use of existing community resources.
• Sustainability: Developing a sustainable model of care that can be fully integrated into the PCN's ongoing services after the initial funding period concludes.
