Summary
The clinician will complete a full biopsychosocial assessment and crisis plan working closely with the MDT to clincially formulate the young persons needs to understand triggers and perpetuating factors that can cause the crisis presentation. The clinician will work in partnership with the young person and their carers along with all involved in the young persons professionals network to coordinate care to formulate safe discharge plans. The clinician will utilise a range of therapeutic skills to engage a young person in accordance with their needs and keep the young person at the center of any decision making when signposting young person to additional service to support in treatment and recovery and build resilience and reduce risk behaviours
