Crynodeb
The post holder will join a small, specialised team, providing all aspects of a community clozapine initiation service across the districts of Bolton, Salford & Trafford.
Assess each referral into the service on a case-by-case basis as to whether they are appropriate for a community initiation/re-titration.
If appropriate, the clozapine team will oversee the entire process of a community clozapine initiation.
If a service user is not able to have clozapine initiated in the community for any reason, then the community clozapine initiation team will arrange for baseline physical observations, baseline bloods, ECG and registration with ZTAS to be carried out in the community prior to inpatient admission to minimise the length of any inpatient admission.
If the patient’s clozapine initiation/re-titration has been started on the inpatient unit, but they are now fit for discharge, the community clozapine initiation team will support the patient with early discharge, so that the clozapine titration can be continued in the community and therefore minimise the length of any inpatient admission.
The clozapine team will support the MHLT’s with the management of clozapine for patients admitted onto Acute Care Trust’s medical wards.
The community clozapine initiation team will co-ordinate daily visits, undertake monitoring and obtain blood samples for patients receiving a clozapine RED result in the community.
The community clozapine team will act as a specialised resource, providing clozapine related advice/support to other services.
