Summary
1. Job Title Practice Nurse2. Responsible to Nurse Lead, GP Partners, Practice Manager3. Job Purpose The Practice Nurse will deliver high-quality nursing care within a primary care environment, providing a wide range of treatment room and long-term condition services. The postholder will work as part of the multidisciplinary team to promote health, manage chronic diseases, support preventative care and contribute to the overall quality and safety of the practices clinical services. 4. Key ResponsibilitiesClinical Duties Undertake chronic disease management reviews, for example diabetes, asthma, COPD and hypertension, in line with current guidelines. Provide treatment room services, including wound care, dressings, ear irrigation, cervical screening, immunisations and injections. Deliver childhood, adult and travel vaccinations in accordance with recommended schedules. Support womens health services such as cervical cytology and contraceptive advice within competence. Provide health promotion and lifestyle advice to support self-management and disease prevention. Triage and manage minor illnesses or minor injuries within scope of practice. Undertake NHS Health Checks and other preventative screening where required. Maintain accurate clinical records using EMIS and follow practice protocols. Ensure safe infection control standards and contribute to clinical risk management. Teamwork and Coordination Work collaboratively with GPs, ACPs, pharmacists, HCAs and the wider MDT. Support continuity and coordination of patient care, including proactive recall and monitoring systems. Participate in clinical meetings, training sessions and case discussions. Contribute to a positive, supportive team culture. Quality, Governance and Service Improvement Engage in quality improvement and audit projects. Support delivery of QOF, IIF and other practice targets. Participate in safeguarding activity and follow all safeguarding policies. Maintain compliance with infection prevention, cold-chain management and clinical governance requirements. Contribute to service development by identifying opportunities to improve patient experience or clinical efficiency. Professional Development Maintain NMC registration and complete required CPD. Participate in annual appraisal and reflective practice. Keep up to date with national guidance relevant to primary care nursing. Support the development of students, HCAs or new staff where appropriate. 5.
