Summary
You will support the Lead Midwife for Governance in delivering operational risk management across the division, ensuring a patient-centered approach that promotes an open and transparent culture and improves clinical safety and quality. This role involves leading the management of incidents, complaints risk assessment, PMRT, and compliance activities within the maternity service, working collaboratively with senior managers and wider governance teams to maintain an effective Clinical Governance Assurance Framework.
You will provide expert midwifery input to ensure timely review and investigation of incidents, oversight of risk management processes, and support for regulatory compliance, including CQC requirements and the Patient Safety Incident Response Framework (PSIRF). They will represent the service at key governance meetings, deputise for the Lead Midwife for Governance as required, and provide line management to the Governance Coordinator.
This role also includes contributing to policy and service development, supporting staff in clinical governance and duty of candour responsibilities, and ensuring that best-practice midwifery standards are maintained across the service. The post holder will deliver annual and strategic objectives through collaborative leadership, role-modelling professional values and behaviours, and supporting the division to meet national, local, and regulatory standards.

1. • Foster an open and transparent culture within maternity services .
• Support staff in meeting Duty of Candour requirements and maintaining ongoing communication with patients and families involved in safety incidents.
• Provide expert midwifery input to support investigating officers and case reviews, ensuring reports are quality assured .
• Maintain the Risk Register and provide regular thematic analysis and performance reports .
• Lead the management, review, and investigation of incidents within the division.
• Ensure timely submission of Rapid Reviews and escalate issues or exceptions appropriately.
• Provide clinical expertise and guidance to the midwifery team.
• .Provide oversight of regulatory compliance, including CQC standards and the Patient Safety Incident Response Framework (PSIRF).
• Attend key governance meetings and deputise for the Lead Midwife for Governance as required.
• Manage and maintain the progression of complaints within Maternity ServicesTo prepare and present deep dives, thematic reviews, MDTs and PMRT cases.
• Act as a change agent, promoting positive behaviours and professional standards across the maternity service.
• Prepare and present reports on risks, incidents, and compliance issues .
• Support continuous improvement initiatives by analysing trends, identifying interventions, and ensuring recommendations are implemented.

The post holder will contribute to policy and service development, support staff in clinical governance, and ensure that national, local, and regulatory standards are consistently met.
