This job description is only a very brief summary of what a PCN Paramedic Practitioner job will entail, and the role will expand and change over time as the needs of the PCN changes, the requirements of the Network DES expands, and the priorities of the population evolve. Primary Responsibilities The following are the core responsibilities of the PCN Paramedic Practitioner. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon other factors such as workload and staffing levels. To holistically assess, diagnose, treat, or signpost patients/service users with undifferentiated or undiagnosed conditions. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. To complete our weekly Care Home rounds and clinically assess patients who have been prioritised for a review based on care home advice and the MDT clinical judgement. Complete annual reviews for patients who are housebound and have chronic long term conditions, with the aim of increasing their confidence to self-manage and improve their health outcomes. Review patients post-discharge who are housebound, or temporarily housebound, and provide a person-centred holistic health assessment of need. Support our Home Visiting Team, if necessary, and provide clinical assessment and management of patients in their own residence. To use advanced clinical skills to provide education to patients, promoting self-care and empower them to make informed choices about their own treatment. Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures. Participate in setting and monitoring standards to ensure effective care and delivery of evidence-based practice. Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working. Ensure clear understanding and utilisation of referral mechanisms within their practice. Prioritise own workload and ensure effective time-management strategies are embedded in own practice. Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. If the post holder has completed an independent prescribing qualification, they will be required to safely prescribe for a range of common, urgent conditions in line with their personal formulary. To identify the limitations of their own practice and liaise with senior colleagues where these limitations have been reached to ensure good quality patient care is maintained.
