JOB DESCRIPTION JOB DETAILS: Job Title: Primary Care Network Occupational Therapist / Nurse / Paramedic Contract 12 Month Fixed Term hours to be negotiated / approx. 24 hours pw Band: 6 / 7 Directorate: PCN Department: PCN Base: Various sites within PCN Responsible for: Supporting the delivery of Population based Proactive Care - usually within patients own homes. Responsible to: Proactive Care Team Lead JD updated: November 2025 Department Core Purpose Taunton Deane West Primary Care Network aims to provide patient centred healthcare. We are dedicated to bringing the general practices together to ensure they are working collaboratively to provide the best service they can for patients. The Primary Care Network has been established alongside Neighbourhood teams to understand and develop services relevant to their local population. There are network priorities that have been set nationally however there is the autonomy to tailor these to local need. The focus is on improving the health and wellbeing of the people who live here as well as taking action to reduce health inequalities. Job Purpose: The post holder will provide a high standard of clinical delivery within the PCN Proactive Care Team. The post holder will provide intervention for the presenting patient from initial clinical assessment, diagnosis, treatment, and evaluation of their care within set cohort groups. The norm is to assess patients within their own home. They will demonstrate safe, clinical decision-making and high-level care for people within the PCN, alongside a strong understanding and use of risk assessments. They will work collaboratively with the PCNs multi-disciplinary team (MDT) to meet the needs of individuals and carers. Duties and Responsibilities Communication and Key Working Relationships Excellent communication and organisational skills Provides and receives complex, sensitive information to patients and carers where there maybe barriers to understanding Be able to work in partnership with colleagues at all levels both within and external to the PCN, to include: PCN Managers PCN clinical and non-clinical colleagues Practice staff AHPP Lead Proactive care clinical lead Neighbourhood teams including Community Rehabilitation Team, Older Persons Mental Health, District Nurses, Social Services, other public and voluntary sector Acute Services Intermediate Care including Rapid Response, Discharge to Assess Adult Mental Health and Learning Disabilities colleagues Children and Young Persons Service 2 Stroke Services Planning and Organisation Ability to manage own time and work flexibly between face to face, and virtual platforms, as required Work with other therapists across the health care systems. Consistently and accurately follow lone working policy procedure Analytics Ability to evidence a sound understanding of the PCN & NHS principles and values Ability to analyse and interpret complex often incomplete information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues Evidence of working across organisational boundaries within health and social care Independent thinker with good judgement, problem-solving and analytical skills Good awareness of national framework, policy and guidelines for Proactive Care. Responsibility for Patient / Client Care, Treatment & Therapy To deliver holistic, comprehensive clinical assessments in patients homes or in the PCNs GP surgeries. To plan, implement, and evaluate Personalised Care and Support Plans - aiming to increase patients wellbeing and to minimise the risk of them requiring episodes of unplanned care. To work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals where appropriate using standardised outcome metrics. Complete timely onward referrals into neighbourhood systems as appropriate. Be able to signpost patients and their families or carers into relevant neighbourhood systems Adhere to professional standards - escalating concerns in a timely manner. To ensure and maintain a high-quality delivery of care that is evidence based, safe and effective To concisely and clearly explain the nature & purpose of Proactive Care to all stakeholders relevant to patient care. To use data, coding and AI systems to enable the effective delivery of upstream, proactive care to the PCN population. As appropriate / depending on profession, complete relevant medical observations Policy, Service, Research & Development Responsibility To comply with legislation and all the PCNs Policies To contribute to and be responsible for implementing any service changes as guided by national guidance. Achieve and demonstrate agreed standards of personal and professional development, actively participate in mentoring and supervision, proactively support Practice Delivery Education thought student/apprenticeship placement. Develop and share learning with colleagues, promoting peer review and best practice 3 within the work environment, alert other team members to issues of quality and risk Participate in relevant audit processes, evaluation and implementing ongoing service change to order to meet population need. Maintain accurate, contemporaneous healthcare records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Ensure adhering to professional standards and code of conduct. To participate in business and professional meetings, as appropriate, and in discussion with your manager, including relevant clinical networks. Responsibility for Finance, Equipment & Other Resources Responsible for the safe use of and provision of adaptive equipment Responsibility for Supervision, Leadership & Management To receive regular clinical and management supervision in an agreed and appropriate format as guided by the Employer. The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis. Information Resources & Administrative Duties The post holder will keep comprehensive notes and records for all patients who have been referred with professional and PCN policy. To complete accurate and timely clinically related administrative duties relevant to the operation of the service and contributing to the review of information needs and systems for data collection To be able to use email for communications across the organisation and PCN sites Any Other Specific Tasks Required The post holder will require access to their own private transport to support visits to patients across the Neighbourhood and travel to different locations within the county when required. They will be required to have appropriate insurance cover for business uses.
