Summary

Sole responsibility of caseload for paediatric continence within Great Yarmouth, Lowestoft & Waveney Input iPM for data collection for Commissioning purposes on a daily basis. Ensure that all patient records are completed and maintained in line with Trustpolicy, observing patient confidentiality at all times. Provide Specialist assessment, support and treatment for children aged 5-16years who have more complex continence needs. Undertake home visits, as required, to give specialist continence input. Be competent in interpreting the combined results from urinalysis, bowel charts and fluid intake & output charts. Be competent in reaching a diagnosis for patients symptoms and treat accordingto treatment pathways. Refer children and young people to other specialists and agencies as appropriate. Work autonomously without direct supervision, accountable for own professionalactions. Refer to the Team Leader if any issues require more specialist advice and intervention, thereby maintaining high service standards. Deal with patient complaints in a positive and sensitive way in accordance withpolicy. Uphold a professional approach and maintain professional integrity at all times. Be aware of the Trust Safeguarding/Child Protection policy and requirements. Gather information to provide specialist advice to assist child protection investigations. Liaise with education staff, social care and other health organisations e.g. paediatricians. Responsibilities for Human Resources Develop, deliver and implement specialist teaching programmes for clients, carers, Health Visitors and School Nurses. Provide theoretical and practical support for teaching staff/practitioners in continence-related clinical skills. Provide line management support for Band 5 continence nurse e.g. conduct PDP, allocate caseload, sanction leave etc. Responsibilities for Physical and Financial Resources Ensure that equipment is used correctly and safely and that it is not operated in contravention of medical device requirements or the Health and Safety at Work Act, e.g.enuretic alarms. Ensure the most effective use of resources is maintained. .Provide specialist continence advice and support for all staff involved in the care of children and young people with continence needs aged 5-16 years, focusing on the need to promote continence where it may be achieved. Provide specialist continence advice, clinical expertise and support to colleagues and care providers within the community and Trust. Act as a role model of excellence in childrens continence care. Provide professional advice, support and leadership to colleagues. Attend, as appropriate, regional and national events to enable Great Yarmouth, Lowestoft & Waveney to be identified as a service provider of childrens continence best practice. Develop and maintain good working relationships with staff supporting the needs of children with disabilities, school nurses, health visitors, occupational therapists and physiotherapists. Ensure that continence health needs are communicated effectively to those eligible to receive the information. Work across acute and community boundaries, working in conjunction with the JPUH Enuresis Nurse Specialist, Constipation Nurse Specialist, Paediatricians, ward and community teams, to meet the needs of children and young people. Policy and Service Development Support the implementation of an integrated continence strategy in line with Department of Health guidance and centres of excellence (ERIC, Bowel & Bladder UK) Support assessors of children and young people with bowel/bladder problems to be aware of, and comply with, related service guidelines and protocols. Development of service guidelines and protocols. Develop clear protocols for assessment of health needs for identified group of children and that activity/quality data is recorded accordingly. Benchmark the service and undertake service review accordingly. Collate result of client satisfaction and compile action plan. Planning and Organising Plan and deliver nurse-led community childrens continence clinics. Develop and maintain health needs assessment that is applicable to the local service and this role in conjunction with key leads and partners. Manage a clinical caseload ensuring effective relationships with other health, education and social care professionals, being mindful of appropriate referral for medical attention. Assume an active role in self-development by undertaking training and updating programmes/courses. Maintain awareness of current development in nursing and medical practice by attending lectures, conferences and seminars as appropriate and by reading available literature. Attend professional meetings as requested by the Team Leader. Plan to deliver training session annually to maintain Trust staff competencies around paediatric continence Analysis and Data Management Maintain patient records and clinical activity recordings to monitor patient contacts and clinical outcomes. Research, Development and Audit Participate in Research and Development programmes as required. Promote evidence-based practice and assist with the setting, monitoring and auditing of standards. Benchmark the service against Good Practice in Paediatric Continence Services benchmarking in action to ensure a quality childrens continence service is being developed/practised. Support the Clinical Governance agenda in the childrens continence service. Develop and undertake clinical audits within the childrens continence service as agreed with the Team Leader. Participate in the audit of delivery of health needs assessment to ensure good quality service delivery and developments.
