Dyma gyfle cyffrous i weithio o fewn y Tîm Lles Meddyliol newydd yng Nghonwy fel gweithwr cymdeithasol. Rydym yn cydweithio’n agos â phartneriaid a gwasanaethau eraill i ddarparu cymorth arbenigol ac ymyraethau ar gyfer pobl sydd ag anghenion cymhleth oherwydd gwahanol fathau o straen seicolegol.
Mae rôl y gweithiwr cymdeithasol yn cynnwys cynnal asesiadau; darparu ymyriadau therapiwtig; ymgynghori â gweithwyr proffesiynol a staff eraill ar y rheng flaen; cynnig cyngor ac arweiniad i ddefnyddwyr y gwasanaeth a’u teuluoedd, gan gynnwys eu ‘cyfeirio’; cefnogi atgyfeiriadau.
Mae hwn yn gyfle cyffrous i weithiwr cymdeithasol â thuedd seicolegol ac sy’n ddiragfarn, sy'n dymuno cadarnhau a datblygu eu sgiliau, gan gynnwys sgiliau asesu, ymgysylltu, therapiwtig a chyswllt proffesiynol. Bydd deiliad y swydd yn cael hyfforddiant a chymorth goruchwyliol ardderchog ac hefyd yn cael cyfle i weithio’n greadigol gyda llawer o benrhyddid proffesiynol.
Mae’n hanfodol bod deiliad y swydd yn gallu teithio ledled y Sir yn rheolaidd, yn aml i ac o leoliadau anghysbell ar fyr rybudd.

Gofynion y Gymraeg: Mae’r gallu i gyfarthrebu yn y Gymraeg yn ddymunol ar gyfer y swydd hon. Rydym ni wedi ymrwymo i'r Gymraeg ac yn falch iawn o’n diwylliant Cymreig. Rydym yn croesawu ceisiadau yn y Gymraeg a’r Saesneg fel ei gilydd ac ni fydd ffurflenni cais a dderbynnir yn y naill iaith na’r llall yn cael eu trin yn llai ffafriol na’i gilydd. Rydym yn frwdfrydig dros eich cefnogi a’ch annog i ddefnyddio’ch Cymraeg, beth bynnag eich lefel; ac mae gennym ni ddosbarthiadau am ddim ar gyfer pob lefel, wyneb yn wyneb ac ar-lein, i’ch cefnogi chi ymhellach.
