Summary
Provide face-to-face triage for patients presenting with a wide range of conditions. Assess urgency and clinical risk, ensuring patients are directed to the right care pathway. Make appropriate referrals to primary, secondary and social care services, following practice (and/or locally agreed) pathways and protocols. Deliver safe, evidence-based advice and support to patients and carers. Work collaboratively with GPs, advanced nurse practitioner and other healthcare professionals. Promote self-care and educate service users to allow them to be empowered to make informed choices about treatment Maintain accurate clinical records in line with NHS standards. Contribute to service development and quality improvement initiatives. Cleaning and maintaining clinical equipment, reporting faults promptly Assisting with the collection and collation of data on needs related to health and well-being and support QOF requirements and data completion Actively participate in seasonal and special projects as requested e.g. flu campaign Ensure adherence to confidentiality, data protection, and information governance requirements Work safely at all times in accordance with legislative requirements and Practice Policy and Procedures
