Summary

Harrow East Primary Care Network (HEPCN) is a group of three friendly and forward thinking practices: Honeypot Medical Centre, Bacon Lane Surgery and Mollison Way Surgery. Our member practices work in close alignment in a defined contiguous locality in Harrow East. We are seeking to recruit a positive, caring and self-motivated individual to take on the role and work with our three Practices. We are dedicated to improving the patient experience and introducing new services that allow us to meet the ever-changing needs of our growing and ethnically diverse population, including a new Enhanced Access service. Over the last four years, the PCN has rapidly matured with the introduction of many new PCN services and the expansion of the PCN workforce, which includes Pharmacists, Physios, Care Coordinators, Social Prescribers, Paramedic, Pharmacy Technician and Digital and Transformation Lead. The Harrow East PCN are looking for an enthusiastic, newly qualified GP (qualified within the last 2 years) - and has not held a substantive/salaried role -to join our team on a fixed term basis of 12 months. The role will be part of the PCN ARRS and may have the potential to be extended beyond 12 months however this is subject to NHS funding and guidance. Main duties of the job Provide face to face and telephone consultations for routine and urgent care. Manage patient care within the Practices, including investigating and reviewing results, developing care plans and making the necessary referrals. Issue and sign repeat prescriptions, adhering to national and local guidelines. Administer and manage patient queries, test results, referral letters and any other duties as required in a timely manner. Make clear consultation notes and records using EMIS. Ensure safe, evidence based and cost effective care in line with Practice disease management protocols and NICE guidelines. Work with our clinical and support teams in delivering holistic and patient centred care. Participate in on-call duties if required. Provide clinical support and guidance to both clinical and administrative staff to maintain the highest care standards.
