Summary

Suitably experienced post-holders will be expected to provide a clinical service in the relevant specialty to patients of the Worcestershire Acute Hospitals NHS Trust and to General Practitioners. Post-holders will be expected to provide a service to patients of the Worcestershire Acute Hospitals NHS Trust, General Practitioners and to attend multi-disciplinary meetings. The successful candidates will be expected to work within multi-disciplinary teams in providing high quality clinical care. It is anticipated work will be undertaken on three sites and closer to the patients home. The Trust is seeking to extend services to GP surgeries/health centres etc and job plans may be revised in due course. Post holders will provide support to the A&E department and acute medicine to ensure timely appropriate care and patient flow through the hospital. Post-holders will provide consultation and advisory services to clinical colleagues in the Trust and Primary Care. Clinical opinion and support for teams in community hospitals: ward round / MDT Clinical opinion and support for community neighbourhood teams Clinical opinion and support for GPs supporting patients in their own homes and in care homes Supporting clinical review of patients who are high users of community and secondary care Face to face / phone advice / email / use of electronic patient records Mentoring generalist clinical staff and supporting enhanced training Encouraging a Comprehensive Geriatric Assessment approach to management of older people living with frailty in both stable and unstable situations Encouraging an anticipatory planning approach to managing acute deteriorations including contingency planning and inappropriate admission avoidance Supporting management and transfer decisions in all care settings ensure right place of care Supporting teams in managing the perceived risks of care outside an acute setting Supporting the education of all those managing Older People living with Frailty and frailty related conditions eg GPs ANPs AHP nurses and care staff. Post-holders will attend cross county & cross site meetings. To collaborate with colleagues to deliver a high quality, timely service and to work towards meeting the Trusts and departmental objectives. Post holders will if relevant, participate in the on call rota to ensure that both emergency and urgent services are provided out of normal hours, to be shared equally with consultant colleagues in post. The trust would expect consultants whilst undertaking on call duties to attend the hospital site to review patients when clinically necessary. The consultant on call must be prepared to attend willingly whenever requested to do so by a member of their medical and nursing team. The post-holder will develop close collaborative links with Clinicians working in his/her area of specialist expertise and work to develop services to patients in these areas to the highest standard, according to departmental protocols and Trust guidance. To include development of diagnostic and where appropriate therapeutic protocols and guidelines, where appropriate this will involve active participation with Primary Care colleagues. The post-holder will participate in the Trusts annual appraisal as described by DOH and leading to revalidation and job planning process. The post-holder will seek approval for any absence from the Trust such as annual leave, professional leave, study leave & other leave and make available this record at your job plan review. Consultants will provide cross cover for periods of annual and study leave.
