Summary
Key Tasks Develop and maintain communications with patients, carers, other professionals and volunteers Work closely with members of multidisciplinary team and outside agencies to ensure the provision of individualised care Demonstrate awareness towards of limits of practice and seek support and guidance as appropriate Escalate any concerns as appropriate Support with telephone advice for patients, carers and other professionals, as required from the Community Co-Ordination centre Promote a safe environment for all patients, carers, visitors and staff in the patients preferred place of care. ~~~Please refer to the attached Job Description and Person Specification for the full duties and responsibilities of this role~~~
