Summary
The role includes coordinating a team of Reablement Support Workers to provide a strengths based person centred approach.
The focus is helping individuals regain or retain their independence, confidence and skills in daily living activities, after hospital admission, illness or deterioration in health.
Duties Included (but are not limited to):
To manage electronic rotas and care planning, working on bespoke IT systems, Word and Excel.
To complete initial assessments, agree goals and review progress with the team.
You will be required to facilitate transfers of care to agencies following a period of reablement and complete comprehensive care act assessments.
Hold duty on a rota basis and make decisions in a crisis.
Undertake formal supervisions with staff and support the Team Manager in management of sickness absence and recruitment and retention of all staff.
Work in partnership with other agencies and professionals building positive relationships.
To have knowledge of Care Quality Commission compliance to ensure high standard of service provision.
Reablement Promotes a culture of independence and has a strong teamwork ethic with job satisfaction.
All training will be given during period of induction.
