About the Service

Our Outreach service supports individuals in the community to live independently and confidently. We provide help with daily tasks like shopping, attending appointments, and managing household routines, while encouraging participation in social activities such as visiting cafes, going to the cinema, and attending local events.

As an Outreach Relief Support Worker You’ll:

>Assist with healthcare needs, including medication and personal care.
>Promote independence by following tailored Support Plans.
>Keep accurate records of support and progress.
>Work collaboratively with colleagues, families, and individuals.

Flexible hours available between 9:00am and 6:00pm, 7 days a week.
