Summary

At Bath & North East Somerset Council we have one overriding purpose – to improve people’s lives. About the role: The Pathways social care team works with adults who have had reablement support following a hospital discharge and are identified to require long term support. The team consist of a Team Manager, Social Workers, social care practitioners and admin staff. We complete holistic Care and Support assessments, Carers Assessments, Support Plans and initial reviews. Working closely with the Hospital Social Care Team, reablement team, and other organisations: health, housing, service providers, and the Community Wellbeing Hub to help meet your identified outcomes. The post holder will manage their own caseload of clients who are currently reserving interim care following a discharge home with the reablement service. You will complete care assessments, support planning, initial reviews and safeguarding. Applicants need to have very strong organisational skills to manage and prioritise their work effectively and in a timely way and will be committed to working as part of a team to support colleagues in delivering a person centred service that is responsive to the needs of BANES residents. We are seeking applications from social workers who have: Thorough understanding of the Care Act 2014, Mental Capacity Act 2005 and Safeguarding Adults procedures. Understanding of NHS Continuing Healthcare (CHC) and hospital discharge pathways. Knowledge of community resources, care and support services and funding arrangements. Experience of working with care management systems including data quality and recording. Experience of completing Care Act assessments, support plans, reviews, Mental Capacity assessments and safeguarding. Experience of multiagency working. Knowledge of preventative, community, technology and a range of support that can prevent, reduce and delay the need for social care services.
