Summary

Clinical/ Operational Responsibilities Operational To support, supervise and act as a role model to trainee nursing associates, health care support workers, students and those new to care roles, review the quality of the care they provide, promoting reflection and providing constructive feedback. To respond to emergency situations of all kinds including violence, cardiac arrest and fire. To maintain effective liaison with all members of the multi-disciplinary team, as well as other agencies who are involved in the service users care. To contribute to team reflection activities, to promote improvements in practice and services. To prioritise and manage own workload, and recognise where elements of care can safely be delegated to other colleagues, carers and family members. To fully adhere and comply with all Trust Policies. Clinical To support the registered nurse for a defined group of service user and effectively liaise with other members of the MDT to ensure a consistent approach to delivering care. To support the registered nurse in assessing and implementing care especially in regard to identifying and managing risk. Contribute to the development of service user care plans by assessing and monitoring signs and symptoms of physical, mental, cognitive, behavioral and emotional distress, deterioration and improvement through a range of assessment tools. Ensure the safe custody of drugs and that they are administered accordingly to Trust policies and the Misuse of Drugs Act and as delegated by the registered nurse. Understand the principles of safe and effective administration and optimisation of medicines in accordance with local and national policies. To recognise the effects of medicines, allergies, drug sensitivity, side effects, contraindications and adverse reactions recognise the different ways by which medicines can be prescribed To safely administer medication to service users as delegated by the registered nurse. Encourage service users to accept an optimum level of responsibility for their individual wellbeing and treatment plan. Deliver strategies to service users as developed by or in partnership with the registered nurse for dealing with disturbed behaviour especially the ability to recognise and respond to signs of imminent disturbance/impending relapse of their mental health condition. Monitor the effectiveness of care in partnership with service user, families, carers and the registered nurse. Document progress and report outcomes. To keep up to date with developments and research in mental health care and applies relevant to practice. To actively contribute to clinical or governance reviews of the teams activity and the monitoring of performance. Contributing to relevant audit practice development on research activity that may be taking place in the work area. Participates in the running of therapeutic service user groups. To be fully involved in presentations in ward rounds, handovers and other meetings where service user care is discussed and reviewed. To participate in the assessment, admission, transfer and discharge of service user according to local guidelines and standards as delegated by the registered nurse. Ensure that as a nursing associate you understand professional confidentiality requirements as identified in the NMC Code of Professional Conduct. To undertake and assist in the review of service user to include: a) Signposting service user and family carers as appropriate; b) Actively linking service user and family carers in with local and/or national services; c) Referring service user and family carers to statutory and third-sector organisations; d) Recording any changes in physical and mental health and reporting these to a more senior members of staff, acting appropriately on the advice given; e) Providing appropriate information to service user and family carers relating to health promotion and their well-being. To maintain an effective working knowledge of the administrative requirements and needs of service user detained under the Mental Health Act (1983). To be aware of their responsibilities as employees under the requirements of the Health Act Safety at Work Act (1974). To be aware of and assist in meeting the psychological, social, physical, recreational and spiritual needs of service user. To recognise and value service user as individuals, recognising the importance of maintaining the service user dignity at all times. To ensure that the care provided respects equality and diversity that is supportive of service user and their carers. To understand and follow the Trust risk assessment policy, including being aware of the need to follow all safeguarding procedures; being aware of the necessity to safeguard and promote the welfare of children, young people and vulnerable adults at all times when working with service users. To have an awareness and understanding of the Mental Capacity Act (2005) and be able to support the registered nurse in carrying out Mental Capacity Act Assessment responsibilities as necessary. To maintain timely and accurate patient written / electronic records under supervision and in line with Trust and service line policy and procedure. To ensure that all complaints or compliments received are dealt with in accordance with Trust policy
