Summary
Job PurposeTo provide a wide range of administrative and support services to Operational teams toensure a consistent Living Well offer is available across the county.The Living Well Officer will work alongside operational teams to develop local knowledgeof existing events and community groups and provide relevant information to teams,health colleagues, district councils, providers, service users and family members.ContextLiving Wellis Norfolks implementation of the3 Conversationsway of working. It is anapproach to social care practice that starts with having open and interestedconversations with people and families needing support. The first conversation aims tobuild links and connections in the community so that people can get back to their liveswithout formal support. The secondwhich may not be neededis used in a crisis toput together an emergency plan without the need for a formal Care Act eligibilitydetermination. The thirdwhich again may not be neededis about putting together alonger-term plan that involves formal support, asking the question, what does a good life look like?This role will help facilitate the Living Well model by providing an initial point of contactand associated support and solutions.The role will also liaise with providers, district councils, agencies and health partnersregarding administrative, contractual and financial arrangements, convening meetingsand supporting Living Well outcomes
