Summary
Job Description Participate in meetings to discuss the needs of allocated clients or service, and prioritise time accordingly. Complete appropriate assessments to inform treatment goals, and plan intervention. Provide individually tailored programmes to meet the needs of the allocated clients. Create engaging activities to improve communication skills of clients. Plan and execute sessions, both 1:1, and with small groups of adults, to develop language, communication and social skills. Provide written reports in a timely manner and/or contribute to MDT reports. Attend annual reviews/other review meetings as required. Ensure timely follow ups are arranged with client to ensure continuity of care for each client. Update the clients patient records after each appointment or activity related to them. Carry out, as and when required, any additional tasks and responsibilities as are reasonably compatible with this job description and its objectives.
