This position offers an excellent opportunity to become part of our team and provide support across the organization.
We pay an attractive salary of £42,839 - £46,142 and offer access to training and development opportunities.
The successful applicant must possess a Diploma / Degree in Social Work or predecessor qualification. If you do not hold the Approved Mental
Health Professional Award you will be expected to undertake this training in order to fulfil the role. Once in receipt of the AMHP qualification
you will be paid a market supplement of £2995 per annum for partaking on the AMHP rota, which will be paid in 12 monthly instalments.
The Assertive Outreach service is based at Hafan Coed in Blackwood and is a multi-disciplinary team that receives referrals from the Community Mental Health Teams on behalf of service users living with complex mental health needs who have difficulties engaging with core mental health services.
The team works intensively with service users at home and in the community, across Caerphilly County Borough. The team works closely with other community and inpatient mental health service areas, other statutory agencies e.g. housing, police, probation, third sector agencies and community resources.
The successful candidate will be required to practice in accordance with National Occupational Standards for Social Work and Code of Practice for Social Care Workers to deliver assessment and care management services. You will need to have a good understanding of current legislation and policy. You will also be required to work closely with carers, other professionals and team members or alone using your own initiative. You will have a varied role and have the ability to deal with crisis situations and will be expected to facilitate safe discharge for service users from hospital into the community.
For the role, we ask that you have:
A Diploma or Degree in Social Work or equivalent predecessor qualification.
Approved Mental Health Professional.
Knowledge of the Social Services and Wellbeing Act 2014, Mental Health Act 1983, related Code of Practice and Secondary Legislation In Wales eg Mental Health (Wales) Measure.
Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business / work purposes to attend meetings.
