Summary

The following are the core responsibilities of the ANP in delivering health services. a. Develop, implement and embed health promotion and wellbeing programmes b. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice c. Implement and evaluate individual specialised treatment plans for chronic disease patients d. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health e. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF f. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children g. Request and collect pathological specimens. Upon receipt, to process and interpret pathology and other test results as required h. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required i. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively j. Maintain chronic disease registers and implement and embed an effective call/recall system k. Develop, implement and embed well woman and well man clinics l. Chaperone patients where necessary m. Prioritise health issues and intervene appropriately n. Support the team in dealing with clinical emergencies o. Recognise, assess and refer patients presenting with mental health needs p. Support in the provision of vaccination programmes q. Be an extended and supplementary prescriber, adhering to extant guidance r. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required s. Contribute to practice targets (QOF, etc.), complying with local and regional guidance a. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.) b. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual) c. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately d. Deliver opportunistic health promotion where appropriate e. Innovation in practice to meet ongoing demands on the practices delivery of care. f. Improve health outcomes for patients by introducing new developments where appropriate. g. Supports practitioners in reporting the practice population to promote health improvement and disease prevention. h. Implement and participate in vaccination and immunisation programmes. i. Communicates confidential and sensitive information to patients, relatives, and carers in relation to their condition. j. Provide highly specialised care advice beyond the scope of normal Nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals). k. Support patients to adopt healthy lifestyles and encourage principle of self-care. l. Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme, National benchmarking. m. To monitor and meet care outcomes against standards and recommend or initiate changes as necessary. There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels In addition to the primary responsibilities, the ANP has the following wider responsibilities: a. Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance b. Support the IPC lead for the practice, ensuring compliance c. Monitor and ensure the safe storage, rotation and disposal of medications d. Participate in local initiatives to enhance service delivery and patient care e. Support and participate in shared learning within the organisation f. Develop an area of specialist interest, taking the lead within the organisation g. Continually review clinical practices, responding to national policies and initiatives where appropriate h. Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures i. Participate in the administrative and professional responsibilities of the practice team j. Accurate and legible notes of all consultations and treatments are recorded in the patients notes k. Ensure clinical systems kept up to date, recording and/or amending accurate details l. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice m. Maintain awareness and knowledge of health care governance, and comply with local and practice policies to include Infection Control, Health & Safety, Medicines Management, Information Governance, etc. n. Maintain awareness and knowledge of local and practice policies regarding Safeguarding Adults, Children, and Domestic Violence and referral where appropriate. o. Be mindful of and act within the Mental Capacity Act 2005 p. Contribute to managerial responsibilities as requested by the Nursing Team Leader q. Attend and participate in practice meetings as required r. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required a. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation
