Summary
Working as part of a multi-disciplinary diabetes team assessing and managing a caseload of diabetes patients maintaining effective communication with patients, carers and professionals to ensure seamless service delivery. Provide expert knowledge of diabetes and work within an agreed job plan to meet diabetes service needs. This should include expert knowledge, understanding and competency in the use of insulin pumps and insulin pump therapy, insulin administration, all diabetes treatments, as well as monitoring devices including continuous blood glucose monitors. Undertake supervised nurse-led diabetes clinics, conducting and interpreting relevant tests and investigations. Work in collaboration with the multi professional diabetes team, supporting medical review clinics, contributing expert, evidence based diabetes knowledge and skills. This should include participating in the joint obstetric/diabetes antenatal clinic to provide diabetes to pregnant women who have pre-existing or gestational diabetes. The consultant led insulin pump clinic and medical review clinics. Excellent and effective communication and interpersonal skills to provide information, explanation and reassurance to diabetes patients in a sensitive, empathetic caring manner. Plan and conduct appropriate follow up of diabetes patients to ensure safe and effective diabetes care delivery.
