Summary
To protect, promote and monitor the safety and welfare of individuals and their carers by holistically assessing individual needs and developing and designing care and intervention plans, treating each person as a unique individual. Wherever possible to promote their individual views and wishes in order to be able to support the individuals rights, by adopting a least restrictive approach. Applies specialist knowledge and analytical skills in supporting service users to make informed decisions to improve the quality of their lives by minimising the impact of their mental illness and psychosocial stressors on their life. To safeguard the lives of vulnerable people, promote recovery focused interventions, which is often in highly emotive situations where there are high levels of complexity, uncertainty, crisis and risk. Provides a social perspective with regard to the causes of islanders poor mental health, and where appropriate, promotes and enables the use of alternatives to hospital admission. Works collaboratively with colleagues to empower service users and carers, maximise service user independence, and ensure the protection of islanders human rights. Takes responsibility for continuing professional development within own area of expertise, with particular regard to the relevant legislation, and the safeguarding of adults and children. Provides timely, accurate, well written reports, in accordance with statutory guidance and Government policy. Provides professional advice, guidance, and training to others as required. The senior practitioners perform their work role in a multi-disciplinary, multi-agency context. It is essential that they have a detailed knowledge of the broad range of professionals and resources they may need to call on, and that they maintain healthy relationships with senior individuals within these services. To lead establishing and maintaining the trust and confidence of individuals and their carers by communicating in an appropriate, open, accurate, transparent, and straightforward way. Clearly explaining service policies and the professional role and responsibilities of the multi-disciplinary team. To be accountable for the quality of their own work and that of those they supervise, take responsibility for maintaining and improving their own, and others, knowledge, and skills, in particular ensuring that the criteria for continuing professional registration are fulfilled. To ensure the team provide person focused evidenced based, least restrictive community care, increase involvement as an alternative to a hospital admission.
