Intro:
Acute hospital discharge and Post hospital pathways, HomeFirst, step down beds. Care Act assessments, MCA’s, Safeguarding knowledge. Requirement to be on site at Royal Sussex County Hospital Brighton.

Key Responsibilities:

Manage complex caseloads in areas such as Child Protection, Safeguarding, and Adult Social Care.

Provide professional assessments, intervention, and support to vulnerable individuals and families.

Work closely with multidisciplinary teams to ensure the best outcomes.

Keep accurate records and ensure compliance with UK legislation and local policies.

Essential Requirements:

Degree/Masters in Social Work and SWE registration.

2+ years of post-qualification experience.

Strong knowledge of UK social care legislation and practice.
