Summary

Promote and maintain a patient-focused and customer-oriented service at all times across the nursing team. Foster professionalism within the unit by motivating, supporting, and empowering all members of the multidisciplinary care team. Facilitate and lead an effective staff communication framework by attending relevant meetings, sharing key information, and ensuring updates are disseminated promptly and accurately. Coordinate daily clinical activity, ensuring effective communication and documentation within the multidisciplinary team (MDT), including consultants, allied health professionals, external agencies, and the executive team, to support safe and efficient care delivery. Promote clear and compassionate communication with patients and their families, particularly when addressing sensitive or emotive topics such as end-of-life care, treatment plans, complaints, or HR-related issues. Provide counselling, reassurance, and emotional support as appropriate. Act as a patient advocate, ensuring patients and their families are kept fully informed of care and management plans through consistent, transparent, and empathetic communication. Identify and resolve communication barriers effectively, using appropriate methods to ensure understanding and continuity of care. Be proactive in managing incidents and complaints, ensuring all verbal and written concerns are addressed promptly and in accordance with Trust policy and procedures. Audit and evaluate nursing documentation regularly, implementing improvements and sharing outcomes. Work collaboratively with colleagues to develop and implement policies, procedures, and quality improvement strategies that enhance patient care. Maintain strict confidentiality at all times and ensure adherence to the Data Protection Act and Caldicott Principles in relation to patient information and data handling. Demonstrate a clear understanding of the legal and professional implications of nursing practice, acting at all times in line with the NMC Code of Professional Conduct and Trust policies
