Summary
The UCR Service provides urgent support, seven-days a week, to help prevent unnecessary hospital admissions. This is achieved by providing a two-hour crisis response delivered by a multi-skilled team of professionals. Support is provided at home or where they usuallyresidefor people who are concerned their health or mobility is declining and are therefore at risk of admission to hospital. Our teams work closely with our local ambulance service to pull category 3 & 4 patients off the ambulance stack, supporting patients in receiving a rapid response in the community by teams with the skills to support them to remain at home.
The UCR service also provide a general virtual ward service, supported by digital technology and remote monitoring. Our multidisciplinary teams ensure that patients receive hospital-level care at home safely and in familiar surroundings, helping speed up their recovery while freeing up hospital beds for patients that need them most. The postholder will berequiredto work autonomously as an Advanced Clinical Practitioner, using these skills to assess, investigate, diagnose,treatand manage patients who may have co-existing and diverse factors making management complex. They will develop,utiliseandmaintainskills beyond the scope of normal practice (for example advanced physical assessment,diagnosticsand prescribing). Within the role there will be a requirement to plan and direct the management of patients including accessing services within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services asappropriate tothe role. The postholder will lead on the strategic development of Trust-wide services in their specialist area, working with other strategic and clinical leads asappropriate. They will also lead andparticipatein education programmes designed to support other clinical staff skills within their speciality.
The postholder will provide peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients. They willassistin the planning, co-ordinating and development of any newly implemented service plans as well asbeing responsible forensuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures.
