Summary


The post holder will be expected to provide detailed specialist smoking cessation advice and knowledge to patients. They will act as a source of specialist advice in the nursing management of patients in their
specialist area who require a high level of judgment, discretion and decision-making whilst as an inpatient/outpatient or in a setting external to the Trust. The post holder will provide education and training to patients, carers and healthcare professionals.

Whilst the post holder's main base of work will be out of Fairfield and Rochdale General Hospitals there may be in the future the need for cross site working in the Northern Care Alliance.
• Undertake in-patient smoking cessation assessments, individually tailoring care as appropriate. Provide specialist behavioural support and pharmacotherapy
• Discuss evidence-based treatments to manage tobacco dependency and support patient quit attempts.
• Work towards achieving Department of Health targets, local targets, relevant CQUINNs and deliver care in accordance with National Institute of Clinical Excellence (NICE) guidelines and the National/local Tobacco Control Plan.
• Act as a resource for knowledge and advice for the multidisciplinary team within the Hospital and community, ensuring ongoing development of effective evidence-based practice through training and education
• Educate patients and their families about the risks of smoking in order to improve quality of life and assist in self-management of problems, with improved health outcomes
• Undertake all administrative duties for the Tobacco Dependency Team, maintaining accurate and professional records, populating referrals and multidisciplinary correspondence
• Represent the Tobacco Dependency Team on the Trust Smokefree
policy
• To effectively and safely manage the patient caseload and direct and support the Band 5 CURE nurse in the team
• Support local and national stop smoking campaigns and work alongside the Greater Manchester Making Smoking History Team
• Deliver adhoc training to multidisciplinary staff across the Trust site to increase and standardise staff knowledge and awareness
