Summary

Act as a core member of YAP team with active presence in YAP Forum meetings. The post holder will carry a small case load of complex and challenging patients with mental illnesses with comorbid neurodiversity in Adult CMHH. To provide day-to-day management to support in delivering community care to service users and their carers based on the intervention-based model of care To work closely with the YAP Lead, Team Manager and other senior To establish strong working relationship with the in-patient Service, Harrow Crisis Resolution Team, Harrow Psychiatric Liaison Team, Early intervention service and CNWL Single Point of Access. To attend the acute services bed management meeting and Lavender walk bed management meeting, once a week to update on CMHH/YAP Lead on actions and feedback to CMHH and vice versa to facilitate any further actions on transfer of care. To work closely with YAP lead to identify the CAMHS cases transitioning to AMHS, sign post in the third sector, help them engage with partner organisations. To facilitate inpatients and HTT transfers in a timely way for complex young people when requested. To offer regular structured supervision to staff within the team, as well as providing informal support and advice to them and to any other team members in the course of their work. To undertake regular caseload scrutiny for categories such as discharge, step down or step up care. To ensuring that required documentation, activity reports and quality standards are maintained within the Teams. To ensure that Trust policies are implemented, including clinical policies, Health and Safety, Fire, Sickness, the reporting of Untoward Incidents and Accidents, and to ensure that the Team Manager is informed of any such incident or any complaint. To assist in the investigation of such matters, at the Service Managers request. To participate fully in HUB MDTs to facilitate care planning for young people where required. To facilitate transfers from Acute inpatients team to EIS and from EIS to CMHH Hub in a timely way for young people between ages 18-25yo To act as Young adults Champion in the CMHH To train and deliver trauma informed approaches and stabilisation manual to the group of young adults on the caseload. To take part in quality improvement projects to ensure high standards of delivery of care are achieved and maintained. To maintain good relationship with VCSE linked with YAP and Peer Support Team. To have an active role with other senior colleagues in the continuing development of a high quality service that is underpinned by evidence-based practice, care pathways and NICE guidance Possess good communication skills and be able to form positive, therapeutic relationship with service users and carers and staff. To be able to communicate sensitively both verbally and non-verbally with service users and maintain good relationships with carers and others. Contribute to Quality Improvement projects Deliver training specific to the role to members of the MDT Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism In delivering your duties you are expected to display behaviours that are aligned with the trusts core values. Make judgement on complex facts and situations which require analysis/ consideration of options. To maintain contemporaneous records of sessions and clinical activity on S1 To participate in yearly CNWL appraisal and continue to develop personal skills by participating in continuous professional development.
