Do you enjoy making a difference to patient care?


We are looking for a dynamic, organised and highly motivated individual to be part of the anticoagulation team and play a key role in anticoagulation stewardship.


You will provide expert leadership, advice and guidance relating to the safe use of anticoagulation, including when patients move between health-care settings or home. The post-holder will work with key stakeholders locally, regionally and nationally to deliver patient safety and system wide initiatives.


You will support the nursing team to operate nurse-led clinics but also have an opportunity to develop your own clinics and participate in consultant-led clinics. An independent prescribing qualification will be required for the role, or a commitment to work towards one.


Previous specialist experience of anticoagulation will be of benefit but is not essential as training can be provided. An insight into managing the risks associated with anticoagulation would be beneficial.


This role is offered as an 8b Advanced Pharmacist Practitioner post; however, there is also the opportunity for you to apply at an 8a level and undertake an agreed development plan to gain the skills and knowledge to progress to an 8b.

• The post holder will be responsible for the provision of a highly specialised pharmacy service to patients across the Trust.
• The post holder will be expected to provide expertise to nursing and medical staff (within the Trust and externally) on the management of individual patients, ensuring the safe and efficient provision of anticoagulation.
• The post holder will be expected to integrate with the relevant clinical teams to ensure efficient transfer of clinical and medicines management issues.
• The post holder will take overarching responsibility for all pharmacy research and audit projects within the service and be responsible for overseeing training including undergraduate placements, trainee pharmacists and diploma including independent prescribing.
• Work with key stakeholders to improve anticoagulation safety within the Trust

Responsible for supporting the safe provision of anticoagulation services at UHL.

Specific responsibilities include:
• To ensure the provision of high quality, safe, comprehensive (clinical, supply, financial and operational) pharmaceutical service to the Trust.
• If a prescriber, to undertake clinics within a specific area
• Clinically screen prescriptions
• Provide expert clinical advice
• Oversee training of staff, including undergraduate placements, trainee and foundation pharmacists, diploma and IP students
