Summary


The Wokingham Community Older Adult Mental Health Team is looking for an enthusiastic, caring and highly motivated Mental Health Practitioner with a relevant professional qualification (Social Worker, Mental Health Nurse or Occupational Therapist) to join our friendly and innovative team.

The post holder will be an experienced practitioner with excellent assessment and communication skills, offering support to older people with mental health needs and their carers: providing advice, information and treatment in the service-users own homes and the clinic setting.

Your base would be Wokingham Community Hospital where we have access to free parking facilities, staff room and a friendly and welcoming team. We work flexibly to meet service needs both from the office and remotely in line with service needs.

Wokingham Memory Clinic is proud to be an accredited Memory Clinic by the Royal College of Psychiatrists Memory Services National Accreditation Programme (MSNAP). Combined with being an ‘Outstanding’, well-led, organisation, and having an ‘Outstanding’ Older Adults Community Mental Health Team in our latest CQC inspection. The post holder would therefore be expected to work towards helping the service maintain these accolades of high standards.

This post is 22.5 hours a week - core hours 9-5 Monday to Friday
We would consider applicants who wish to work these hours across 4 or 5 days.

The post holder will also offersupporttoolder people with mental health problemsandtheircarers,providing advice, informationandtreatment in the service-users own homesand the clinic setting. Youmay be asked to work across the widerservice;however, youwillprimarilybejointlyresponsible for patients on the Memory Cliniccaseload. You willwork collaboratively with the other Memory Clinic practitioners, the widerteamandother disciplines from both statutory and voluntary sectors,as well as contributing towardsthe development of the service.


The post holder will:

• undertake comprehensiveassessmentsina range of settings includingthepatient’s own home

• Provide theappropriate clinicalfollow upwith training and support

• Identifyand select from a range of health, social and voluntary agencies which couldassistand improve the care of individuals and their families.

• Bejointlyresponsible for the Memory Clinic caseload, meeting the needs ofour patientsand carerswhilstworking collaboratively with other Memory Clinic practitionersand wider service.

• Monitor and evaluate care plans,making adjustmentsas appropriate

• Haveexcellent problem-solvingabilities.

The “must haves” for you to be considered for this role: 
• Have the relevant professional qualification (Social Worker, Mental Health Nurse, Occupational Therapist,)
• You must be enthusiastic, caring and highly motivated with a passion and understanding for working with Older People
• You will be an experienced practitioner with the relevant mental health / clinical experience.
• Be a car driver with the ability and readiness to travel.
