We have an exciting opportunity for individuals to join us and be part of our new Home Assessment Team, putting people first and ensuring they get access to the right supports at the time when they are ready to return home from hospital care.

As a key member of the team, you will work closely with people, their families and colleagues across Health & Social Care to ensure we deliver good quality support in those first days after they return home, improving both their health and wellbeing and enabling them to live safely within their own homes.

You will join a team who deliver a person centred, responsive and flexible service which meets the needs of people and their families. You will work closely with the Home Support Team Leader to ensure that people are comfortable and happy, living independently and are safe within their own homes and communities.

We are looking for motivated and enthusiastic individuals with a pleasant, friendly manner who can be respectful of the privacy and dignity of individuals. As a good communicator, you must also be able to observe and respond to changes in the health and wellbeing of people in your care, updating and recording individual care plans as appropriate. Previous relevant experience working within a care setting and undertaking moving and assisting is desirable together with some IT skills.

A SVQ Level 2 in Health & Social Care or equivalent is desirable, successful applicants who don't currently hold this qualification will be supported to achieve this.

A driving licence is essential for this post.

You will require to become a Protecting Vulnerable Groups (PVG) Scheme member prior to commencing in this role.

You will also be registered or eligible for registration with the Scottish Social Services Council (SSSC).


2 Week Work Pattern :
Week 1 - Monday, Tuesday, Friday, Saturday and Sunday
Week 2 - Wednesday, ThursdayWorking here at North Lanarkshire Council
