Summary
Patient Care: Carry out assigned tasks to meet the care needs of the people we support. To work under the direction of the registered professional to allocated patients following the plan of care. Contribute as part of the multi-disciplinary team towards the implementation of care plans. Undertake a person-centred approach to meeting the health needs of people with a learning disability. To work as part of the multi-disciplinary team but will visit as a lone worker as appropriate, feeding back to the registered professional. To be aware of the risks, risk assessment and feedback any changes to the registered professional. Support registered professionals and the multi-disciplinary team to collate information to inform their assessment. To be responsible for their designated work on, a daily basis as agreed with the registered professional. To overcome barriers to communication using a range of communication methods, e.g., Makaton. To adapt information into a format that is accessible for people with a learning disability. Awareness of how to diffuse situations of aggressive and abusive behaviour guided by care plans, risk assessments, good practice and established guidelines, and report to the registered professional. To support access to health appointments following the nurse care plan in relation to desensitisation, reasonable adjustments and anxiety management. To encourage interest and engagement in a variety of daily activities, including self-care, domestic skills, productivity, and leisure Establish and maintain good relationships with service users, carers both paid and unpaid and staff across multi-agency boundaries. Have an awareness of, and be able to, recognise safeguarding concerns and escalate these to a registered professional. To have knowledge of Positive Behaviour Support approaches The post holder may be asked to undertake duties not directly highlighted within this job description. This will only be done when the skills and experience of the post holder meets the requirements of the role. Leadership To work without direct supervision but under the plan of care of the registered professional. To work collaboratively with all relevant parties to promote effective team working. To act as a role model and participate in supporting new or less experienced staff and student nurses. To know when to escalate issues or concerns to the appropriate person. Strategy and Service Development Adhere to Trust policies and procedures relating to professional conduct and standards. Financial Resources/Management Be able to self-manage time/work demands. Effective time management and organisational skills Ability to manage competing demands. Information Resources/Information Systems To keep accurate and up-to-date records of in accordance with the Trust Record keeping standards and national standards. Ability to communicate through both electronic and written form. Ability to use electronic systems as per organisational requirements. Education To attend mandatory training courses and relevant training as required to meet individual needs. To attend training identified through an annual appraisal as documented on a personal development plan. Partake in lifelong learning and commit to ongoing training.
