Summary
Mae Tîm Gwaith Cymdeithasol Pobl Hŷn ac Ysbyty o fewn Gwasanaethau Oedolion Conwy yn gofyn am Weithiwr Cymdeithasol llawn amser.
Dyma gyfle cyffrous i weithio o fewn Tîm Adnoddau Cymunedol yr Bae Colwyn sydd yn dîm amlddisgyblaethol wedi’i hen sefydlu. Bydd y deiliad swydd yn gweithio gyda Phobl Hŷn a’u teuluoedd/gofalwyr a hefyd pobl wedi eu rhyddhau o’r Ysbyty yn ddiweddar. Er mwyn bod yn gymwys i wneud cais rhaid i chi fod â gradd mewn Gwaith Cymdeithasol neu gymhwyster gwaith cymdeithasol cyfatebol, fel DipSW. Yn ddelfrydol bydd ymgeiswyr wedi cael profiad ar ôl cymhwyso mewn gweithio gyda Phobl Hŷn, teuluoedd a gofalwyr.

The Older People and Hospital Social Work Team within Conwy Adult Services require a full time, Social Worker.
This is an exciting opportunity to work within the Colwyn Bay Community Resource Team which is a well-established multi-disciplinary team. You will be working with Older People and their families/carers and also people who have recently been discharged from hospital. You will use a collaborative approach with a strong focus on outcome based planning and reabelement.
Support will be available through regular formal supervision and day to day informal supervision, a variety of supportive group sessions and excellent training opportunities.
In order to be eligible to apply you must hold a degree in Social Work or an equivalent social work qualification, such as a DipSW. Ideally applicants will have post qualifying experience in working with Older People, families and carers.
It is essential that you have the ability to travel throughout the County on a regular basis, often to and from remote locations at short notice.
Due to the nature of the work, it will be necessary to obtain a satisfactory disclosure from the Disclosure and Barring Service (DBS).
This role offers flexible working options for a work life balance. This can include adjusting your working day and hybrid working, ie a balance of office and home working.
