About the Role
The London Borough of Bexley is seeking an experienced Adult Social Worker to join the Hospital Integrated Discharge Team (HID). This is a 3‑month temporary post created to support a new one‑off project.
You will play a vital role in facilitating safe and timely discharges from hospitals, mental health units, and rehabilitation centres, working closely with health colleagues and community partners to ensure adults at risk receive the right support.
Key Responsibilities
Facilitate safe discharges in line with statutory guidance and hospital discharge pathways.
Complete holistic assessments for service users and carers, ensuring dignity, choice, and independence.
Develop personalised, strength‑based care packages that promote independence and cost‑effective solutions.
Undertake safeguarding duties in line with policy and procedures.
Collaborate with health colleagues, voluntary services, and multi‑agency partners.
Provide timely reports and documentation, contributing to service development and team objectives.
Requirements
Qualified Social Worker with relevant registration.
Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005.
Experience in hospital discharge, safeguarding, or adult social care settings.
Ability to manage complex caseloads and deliver person‑centred solutions.
Enhanced DBS and compliance documentation required.
