Summary

The Patient Pathway Tracker will work within the central cancer team and is responsible for the timely tracking of cancer patients through referral, diagnosis and treatment. You will use various IT systems, be expected to understand the national and local targets affecting Cancer Access and be able to manage pathways efficiently in accordance with these timescales.
You may be expected to assist the MDT Coordinators, Clinical Teams, Cancer Managers and other divisional managers to ensure that the specialty is able to deliver safe and effective services for patients. You should have a keen eye for detail, be able to enter data accurately onto the relevant hospital systems and ensure patient records are kept up to date at all times.
This is a hybrid working role and you will be required to work from home and onsite.
If successful, you will be responsible for the prospective monitoring, reporting and tracking of cancer patients, ensuring that the patient’s journey is processed in a timely and efficient manner, in line with the Cancer Waiting Times Targets and Cancer outcomes and services dataset guidelines. This includes:
• Ensuring that systems are in place, so that all newly referred and diagnosed cancer patients are identified and reported on.
• Navigating patients through their care pathway, from point of referral to treatment. Using hospital IT systems of reporting including PAS systems, endoscopy reporting system, RMS and histology.
• Being responsible for identifying and resolving delays in the patient pathway. Where this is not possible, to follow the escalation procedure, highlighting any relevant issues to line Management.
• Highlighting any bottlenecks within the process, communicating regularly with line management.
• Escalating diagnostic appointments for target patients, (if booked outside the appropriate timescale).
• Liaising with other Trusts both within the Network and outside, in order to achieve continuous monitoring of the patients pathway.
• Identifying and advising clinical teams on when waiting times adjustments can be made to the patient journey, ensuring these are accurately documented.
