The aim is to deliver person-centred, trauma-informed care for people with moderate to serious mental health needs who often fall between primary and secondary care.

Your new role
As a Recovery Navigator, you will play a vital role in supporting individuals with moderate to serious mental health needs who often fall between primary and secondary care services. Working as part of the Mental Health and Wellbeing Integrated Network Team (MINT), you will help bridge gaps in care by delivering trauma-informed, person-centred support that empowers people to improve their wellbeing and resilience.

This role involves building strong relationships with service users and professionals, coordinating care, and signposting to community resources. You will work collaboratively with GPs, social prescribers, and voluntary sector organisations to ensure individuals receive holistic, integrated support tailored to their cultural and social needs.
Responsibilities:

  • Deliver personalised mental health support and interventions in line with recovery principles.
  • Complete support conversations to identify needs, goals, and desired outcomes.
  • Work collaboratively with GPs, social prescribers, and other health and social care professionals.
  • Signpost and connect clients to community resources and voluntary sector services.
  • Maintain accurate and timely records in line with organisational policies.
  • Manage safeguarding responsibilities and promote a 'Think Family' approach.
  • Support clients with wider needs such as housing, debt advice, and employment.
  • Participate in daily handovers and multidisciplinary discussions within the MINT hub.
  • Promote inclusion, challenge stigma, and advocate for clients where appropriate.
  • Contribute to service development by sharing feedback and best practice.
