Summary


To provide community nursing support to young adults who have a learning disability during the transition from children to adult services. The post holder will work across a variety of community settings to deliver nursing assessment, intervention and review to individuals who may have complex presentations. Participate in research and development activity and contribute to the overall development of learning disability services. Provide supervisory support to junior nurses and unqualified members of the team.

The role will include working in a multidisciplinary manner to support the transition from children's service to adult services, this will include visiting people in their education and home settings
