Summary

Aspects of the role include: To assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patients diverse needs, their carers and other professionals Deliver interventions that are evidenced basedBe responsible for the safe monitoring, management and administration of medication Work autonomously, building leadership skills and advanced clinical assessment skills.You will build effective relationships with patients, their carers, family and colleagues across the MDT. You will develop exceptional planning and organisational skillsYou will be able to analyse care and treatment provided, and develop sound clinical decision makingYou will be trained to deliver high quality care and develop. Knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, understand bladder and bowel dysfunctions, provide supportive care to carers and family membersYou will delegate and support junior staff, building leadership skills A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. Please note that this post does not meet the pay or skill level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary / skill level is below the minimum required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
