Summary

The main responsibilities of the postholder will include; supporting people affected by cancer with their health and wellbeing on a 1:1 or group basis. Ensuring joined up care by liaising and working alongside members of the multidisciplinary team working across the community, acute and primary care. Give accurate and appropriate information and signposting to known groups and organisations to people affected by cancer both locally and nationally. The post holder will be fully supported by Cancer Specialists and will work within an agreed competency framework and protocols to carry out the following duties: Keep accurate and timely records of all care, advice and support delivered. Recognise if problems occur during an episode of support and alert the appropriate professional staff. Be accountable for aspects of own practice. Ensure that those aspects of service, delivered as part of remit, meet local quality standards. Ensure that the dignity and respect of service users is maintained. Be proactive in maximising health, well-being and social inclusion of carers, partners and families. The post holder will ensure that all contact with colleagues, users of the Next Steps team and organisational partners will be professional with a positive organisational image demonstrated at all times. The post holder will put patients first in all that they do, valuing all and will seek to understand peoples aspirations, priorities, needs, abilities and limits. Administratively the postholder will ensure: All urgent and/or confidential communications are received and distributed from/to relevant parties in a timely manner. Smooth running of health and wellbeing events for the team, including arranging and changing appointments, prioritising these as appropriate, plus telephoning patients to organise, change or rearrange appointments. Organisation of their own clinical case load, striving to deliver quality patient experience each time. Provide high quality administrative support for the Next Steps Team regularly, assisting when necessary with the ordering and distribution of correspondence, information leaflets and clinical products. Support Next Steps team members to collect evidence and information as part of the Gloucestershire Health and Care Services NHSFT, Macmillan Cancer Support compliance monitoring.
