Summary

As a Student SCPHN you will be required to: Comply with and fulfil the objectives of the School Nursing Team Participate in the setting of personal objectives with Lead Nurse for Development and Training (School Nursing) through the job appraisal and clinical supervision processes Demonstrate nursing procedures where applicable Engage fully with allocated university Complying with SWBHs policies, procedures and guidelines and with relevant statutory/ Department of Health directives Ensuring data is inputted onto systemone accurately and on time as required by SWBH. Develop and maintain effective communication networks within the service and with other health professionals, statutory and voluntary agencies Accurately and timely recording of all care given to the patients and report any changes in the patients condition to the general practitioner or other members of the health / social care team. Attend and participate in staff meetings, case conferences and other meetings as required Identify the multiple needs of the patient, family / carer giving counsel and advice appropriate to the situation Use the knowledge and skills necessary to assess individuals and groups in an holistic way taking into consideration cultural differences Work within the primary care setting as a member of the health care team, participating in activities, which address the health needs of the caseload Provide and maintaining a high standard of care for patients in their homes, health centres and practice surgeries using a model of care, that is consistent with best evidence, within own scope of practice and legislation Identify, plan and undertake specific health promotional activities with identified target groups Ensuring that concerns and identified potential risks are referred / escalated appropriately Please see the posts JD for full information
