The Field Supervisor responsibilities include but are not limited to the following:
Undertake initial visits to new service users to set up a care package ensuring all paperwork is completed including all risk assessments etc.
Explain Paperwork and service to clients.
Undertake care reviews in association with other professional colleagues such as Occupational Therapist
Attend service reviews and/or care conferences with social workers.
Highlight any care package amendments required to Haringey Care Co-ordinators and OT
Conducting/supporting initial assessments and assessments for existing clients
Introduce new carers to clients and train new cares in the practical delivery of care.
Undertake care visits when required, e.g. due to staff sickness or when new service users have not yet been allocated a care team and provide out of hours “on call” cover in rotation with other senior staff. A percentage of total working time of at least 50% will be dedicated to providing direct care visits.
Develop positive relationships with all agencies involved in the delivery of care to the service users in their patch and seek to further develop the level of business undertaken by The Company in their area.
Conducting spot checks/field care supervisions monthly or as frequently as required
Identifying any concerns during spot checks and working together with HR/Care Co-ordinators for improving performance or taking the necessary action
Staff Training & Supervision
Introduce carers to new clients.
Train new carers in practical care tasks
Supervise carers as and when required
Taking responsibilities for MAR Charts collection and analysis
Responsible for the supervision of carers across all fields
Undertake care visits where necessary i.e.in event of staff sickness.
Provide on call cover in conjunction with other senior staff.
Quality Assurance and Communication
Support QA Manager as required
Attend staff/management meetings as required.
Ensure Care Co-ordinators are aware of resource issues so that recruitment can be planned.
Bring to the attention of Care Co-ordinators any complaint made by a service user
Be proactive in reporting back to Lead Care Coordinator/Operations Director of any issue effecting the delivery of care.
