Summary

Responsibility for Patient/Service User Care 1. To support patients/service users in undertaking activities of daily living within careplans. 2. To monitor and report changes in patient/service users presentation to the staffmember in charge and take appropriate action. 3. To follow all relevant policies and procedures. 4. To report issues of concern in line with policy and contribute to any Quality and Safetyprocesses. Communication 1. To build a rapport with patients/service users and their support network,communicating with them in a way that respects their views. 2. To use a wide range of communication skills that can be adapted to meet the needsof complex and challenging situations. 3. To demonstrate effective communication skills with all members of the multi disciplinary team. 4. To provide clear, concise, and accurate electronic and paper records of the care givenin accordance with agreed policy and standards. 5. Communicates to patients/service users and relatives compassionately, in aconfidential and sensitive professional manner. 6. To acknowledge and respect language and cultural differences of patients/serviceusers, their support networks and colleagues. Leadership 1. To promote high quality care through the role modelling of professional behaviours. 2. To advocate on behalf of patients and service users 3. To take accountability for own decisions and actions. Planning and Organisation 1. To organise own workload and tasks, prioritising as appropriate. Partnership Working 1. To demonstrate an understanding of and apply the principles of the CARE Values. 2. To contribute to effective team working through communicating with others in aconstructive way offering support and assistance and making suggestions for the improvement of care delivery. 3. To attend and participate in team meetings. 4. To understand the role of a range of professionals from other service areas who maybe participating in the care of a patient/service user. Responsibility for Physical and Financial Resources 1. To use public funds and equipment responsibly and appropriately. 2. To use consumables appropriately, monitoring stock and minimising waste. 3. To handle valuables in accordance with agreed policies and procedures. Professional Development 1. To undertake the Care Certificate and Band 3 Competency Pathway within a specifiedtime scale. 2. To complete training in accordance with the Statutory, Mandatory and Essential toRole Training Policy. 3. To participate in supervisory one-to-one meetings and HSCs Values Based Appraisalprocess, developing plans to meet personal and organisational objectives. 4. To demonstrate the ability to develop new knowledge and skills, applying this topractice, and share with colleagues as appropriate. 5. To participate in the induction, orientation and development of new staff, studentsand any voluntary workers.
