Summary
Purpose of the role To deliver safe, evidence-based nursing care within a multidisciplinary primary care team, supporting patients across South East London. The Practice Nurse plays a key role in managing long-term conditions, promoting preventative care, and contributing to clinical quality frameworks such as QOF and local enhanced services.Key responsibilities Manage long-term conditions (e.g. diabetes, asthma, COPD) through routine reviews and proactive care. Run chronic disease and health promotion clinics. Provide cervical cytology, immunisations (childhood, adult, travel), and seasonal vaccinations. Deliver wound care, contraception, and sexual health support. Support QOF and local enhanced services through accurate documentation and proactive care. Promote health education, self-management, and preventative interventions. Maintain high standards of infection control and clinical documentation. Collaborate with the wider clinical team to deliver joined-up, patient-centred care. Contribute to service development and continuous improvement initiatives.
