Summary
Conduct support conversations using tools like DIALOG+ to create tailored, strengths-based support plans. Deliver person-centred, culturally sensitive, and trauma-informed interventions. Use your own lived experience (if applicable) to provide hope and connection. Support individuals with navigation around housing, employment, relationships, and mental wellbeing. Maintain accurate and timely casework, risk assessments, and outcome monitoring. Promote recovery, inclusion, and equity in everything you do.
