Summary
1. We are looing for 4 sessions per week. Consultations and where appropriate, physical examinations for the purpose of identifying or reducing the risk of disease or injury. 2. The doctor must be available at agreed times during the agreed sessions per week for surgery consultations and telephone advice. 3. Consultation is all face to face in the surgery. The doctor will be expected to carry out a fair share of the home visits on days worked. 4. Advice to patients concerning general health, and the significance of diet, exercise, smoking, alcohol consumption and drug misuse. 5. Arranging vaccination or immunization against infectious diseases in accordance with national guidelines and policies. 6. Referral where appropriate for the provision of any other services under the NHS Act. 7. Referral to other agencies e.g. Social Services where appropriate. 8. Contraceptive Services in accordance with Para 29 of the National Health Service (GMS) Regulations 1992. 9. Maternity Medical Services in accordance with Para 31 and Part II of Schedule 5 of the National Health Service (GMS) Regulations 1992. 10. Other clinics according to the interests of the post-holder. 11. Hold a personal list of registered patients to maintain continuity of care. 12. Opportunity to be involved in extended access clinics. 13. Be involved in care home LES in supporting our care home residents on a rotational basis 14. On-call commitments on pro-rata basis. 15. Participate in practice educational meetings, mandatory training requirements and maintain CPD for annual appraisal and revalidation.
