We are looking for a skilled Clinical Lead to manage our Community Nursing Team based in the Dover area.
Our Community Nursing Team’s provide interventions for housebound patients aged 18 and over delivering personalised nursing care to those with complex and long-term conditions. Our aim is to enable our patients to stay at home and prevent avoidable hospital admissions.
Members of our team are supported to develop advanced skills to manage the complexity of patients on the caseload through a variety of training opportunities.
As an experienced Community Nurse, you will be able to provide your team with sound clinical judgement and act as a role model for your profession.
You will be accountable for your team’s activity, undertaking or delegating the triage of new referrals, coordinating the care delivery and allocation of work to members of the team with the appropriate competencies. Taking responsibility as the senior clinician you will be a problem solver and decision-maker, assessing complex patients and planning their care needs.
You will be responsible for responsible for ensuring appropriate allocation of staff resources on the duty roster, managing annual leave and ensuring staff remain compliant with mandatory and essential training. There is a need to ensure that capacity and demand for the service is managed effectively and concerns are escalated in the appropriate manner.
The Community Nursing service is offered to all housebound adults, daily including bank holiday and weekends so the shift patterns cover 08:00 until 18:00; however this role is primarily Monday to Friday with flexibility required to meet the needs of the team and the role.
