We are looking for a highly motivated Community Health Care Assistant to join our community nursing team based in Central Norwich.

We are looking for a highly motivated Community Health Care Assistant to join our community nursing team, covering the service seven days a week. This role is key to the on-going development of the locality. You will eventually be working in an autonomous capacity seeing a group delegated patients in their own homes. Current experience working in a community setting would be beneficial but is not essential.

Your role will be key to integrated working within the community team which consists of community nurses, assistant practitioners, other health care assistants and phlebotomists. The successful applicant must demonstrate that they can undertake appropriate assessment of patients in their own home, manage care plans and deliver treatment within a multidisciplinary team. This is an exciting opportunity to enhance your skills with the support of experienced nursing staff.

IT skills will be essential within this role. Training in IT systems for this role will be undertaken as part of a comprehensive induction programme. You will be working in a team setting and would be expected to feedback any concerns to senior members of the team and be an effective team player.

The post holder will need:
• To work generically in provision of care, to patients under the supervision of a qualified practitioner, following a prescribed treatment/care plan in order to prevent admission to hospital.
• Plan and prioritise own visits to delegated patients.
• Work within clinic environments with supervision of a qualified practitioner.
• Ensure patient held records are completed for each visit/interaction, and that SystmOne inputting is completed on a daily basis.
• Work predominately independently on specified tasks, making basic judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified practitioner.
• Liaise and support other health care professionals as required.
• Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy, undertaking health promotion / education as required, e.g. smoking cessation, falls etc.
• Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
• Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. taking blood glucose results, wound care, administration of medication such as insulin.
• Be responsible for ensuring equipment is maintained and used safely, following relevant training.
• Demonstrate an awareness and understanding of the importance of consent.
• Acknowledge and recognise peoples’ expressed beliefs, preferences and choices.
• Share responsibility of indirect patient contact tasks, such as, answering telephones, arranging appointments, processing referrals and inputting activity data.
• Share responsibility for maintaining store cupboards, cleaning specialist equipment and general housekeeping tasks.
• May be exposed to bodily fluids, infected material and blood products, therefore must utilise universal precautions and adhere to infection control policies.
