Summary

We are currently looking for an Adult's Social Worker to join a Reviews Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

The team deals with vulnerable adults in the community to review existing care plans, packages and placements to ensure the right level of support and funding is given. Responsibilities include assessing service users under the Care Act 2014 as well as bringing cases to commissioning panel when necessary. The team is highly proactive and ambitious to ensure all packages are reviewed and adjusted within a reasonable timescale.

About you

A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years of experience working in Adult's community based teams is essential for this position. The successful candidate will have recent and comprehensive experience completing learning disability placement reviews.
