We are now looking for a Care Coordinator to join our dedicated team in Colchester. This is a key role responsible for ensuring that high-quality, personalised care is delivered consistently to every client, every day.

Job Purpose

The Care Coordinator plays a vital part in the smooth running of our services by organising, scheduling, and monitoring the delivery of home care visits. You will work closely with clients, families, and care professionals to ensure continuity of care, high standards, and compliance with CQC and company policies.

Main Responsibilities
Care Coordination and Scheduling

Prepare and manage weekly and monthly rotas to ensure clients receive consistent care from regular care professionals.

Schedule new care packages and allocate staff according to their skills, experience, and client preferences.

Communicate any rota changes promptly to staff and clients, ensuring transparency and continuity.

Manage on-call and out-of-hours cover on a rota basis to maintain 24/7 service delivery.

Monitor Electronic Call Monitoring (ECM) systems to track visit times and ensure service compliance.

Staff Support and Engagement

Maintain close and supportive relationships with care staff, conducting regular check-ins and supervision sessions.

Support recruitment by identifying staffing needs, attending interviews, and assisting with induction and onboarding of new staff.

Ensure all staff have up-to-date mandatory and specialist training, including infection control, safeguarding, and dementia awareness.

Promote a positive team culture that reflects Diana’s Quality Care’s values of compassion, respect, and excellence.

Quality and Compliance

Work with the Registered Manager to ensure all care delivery meets CQC standards and company policies.

Maintain accurate client records, risk assessments, and care plans using digital systems.

Record and report incidents, safeguarding concerns, or changes in client condition promptly.

Conduct spot checks and quality assurance visits as required.

Client and Family Liaison

Serve as a first point of contact for clients and their families, handling queries or concerns with empathy and professionalism.

Support care reviews and contribute to ongoing care planning and service improvement.

Foster strong relationships with external professionals, including district nurses, social workers, and GPs, to ensure holistic client support.

Administrative Duties

Maintain accurate records for scheduling, invoicing, payroll, and compliance.

Ensure annual leave and sickness are managed to maintain service continuity.

Provide detailed handovers to the out-of-hours team and management as required.

Qualifications and Experience

Essential:

Previous experience as a Care Coordinator or in a similar role within domiciliary or residential care.

Excellent organisational and communication skills.

Strong IT literacy, including care management or scheduling software.

Ability to work independently, manage competing priorities, and remain calm under pressure.

Demonstrated understanding of CQC standards and compliance requirements.

Desirable:

NVQ/QCF Level 3 in Health and Social Care (or willingness to work towards it).

Experience in coordinating dementia or palliative care packages.

Full UK driving licence and access to a vehicle.
