Due to the expansion of North Ayrshire Health & Social Care Partnership within Crosshouse Hospital, we are seeking a motivated and compassionate Occupational Therapist to join our newly developed Discharge to be Assessed model of care. This is an exciting opportunity to play a vital role in supporting patients to transition safely and effectively from hospital to home, ensuring care is delivered in the right place at the right time. As part of the multidisciplinary team, you will provide timely assessments, interventions, and recommendations that promote independence, prevent unnecessary admissions, and support patients in achieving their goals in the most appropriate environment. Key Responsibilities • Undertake functional assessments for patients referred to the D2A pathway. • Work collaboratively with patients, families, carers, and health & social care colleagues to plan safe and effective discharges. • Deliver person-centred interventions that promote independence, safety, and wellbeing. • Contribute to timely discharge planning, avoiding delays in patient flow. • Document clinical decisions and care plans in line with professional standards About You • HCPC-registered Occupational Therapist with evidence of continuing professional development. • Skilled in functional and environmental assessment, equipment provision, and discharge planning. • Effective communicator with the ability to work collaboratively in a fast-paced multidisciplinary environment. • Committed to promoting independence, dignity, and high-quality patient care.
